Names of all members in a family unit
Social Security Numbers and Dates of Birth for all
members of Family Unit
Copy of Insurance Card with Policy Number
Name of Coverage Provider
Copy of Insurance Certification to include:
Cover individuals
Months covered...
Beginning coverage date
Ending coverage date – if it applies
Copy of Divorce Decree
If living outside the United Sates, information to
substantiate residence and
number of days living outside the US
Form 1095-A – required to complete a Federal Income
Tax Return if insurance purchased from the Marketplace
